Semaglutide vs. Tirzepatide: How Do They Compare?

COMP-000001

Direct Answer

Tirzepatide (Mounjaro, Zepbound) and semaglutide (Ozempic, Wegovy) are different active ingredients from different manufacturers. In a direct head-to-head trial, SURMOUNT-5, tirzepatide produced significantly greater weight loss than semaglutide at 72 weeks, 20.2 percent versus 13.7 percent. Tirzepatide's dual GLP-1/GIP mechanism is the leading explanation for this difference.

The Core Mechanism Difference

Semaglutide activates only the GLP-1 receptor. Tirzepatide activates both GLP-1 and GIP receptors, a dual mechanism that appears to produce a stronger combined effect on appetite and blood sugar than targeting GLP-1 alone.

What the Numbers Actually Show

Beyond the SURMOUNT-5 head-to-head result, individual trial data tells a consistent story: semaglutide's STEP 1 trial showed about 14.9 percent average weight loss at 68 weeks, while tirzepatide's SURMOUNT-1 trial showed up to 20.9 percent at the highest dose over 72 weeks. These aren't identical trial designs, so some caution is warranted comparing them directly, but the head-to-head SURMOUNT-5 result is the most reliable direct comparison available.

Where Semaglutide Has the Edge

Semaglutide has a longer approval history and a broader base of long-term cardiovascular outcome data, including established cardiovascular risk reduction. Tirzepatide's cardiovascular data is accumulating but has a shorter track record simply due to being newer to market.

Side Effects

Both share a similar gastrointestinal side effect profile, nausea, vomiting, diarrhea, most common during dose escalation. Some data suggests tirzepatide may carry a slightly different distribution of specific GI side effects compared to semaglutide, though individual tolerance varies significantly.

Which One Might Your Doctor Prescribe

This depends on your specific health profile, insurance coverage, and treatment goals, not simply which drug performed better in a trial. Discuss both options directly with your provider.
