Tirzepatide is a first-in-class dual GIP/GLP-1 receptor agonist, available under two brand names with distinct approved uses:


- Mounjaro® — improving blood sugar control in adults with type 2 diabetes, alongside diet and exercise; also indicated to reduce the risk of cardiovascular death or worsening heart failure in adults with heart failure with preserved ejection fraction (HFpEF).

- Zepbound® — chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI ≥27) with at least one weight-related condition; also approved to treat moderate-to-severe obstructive sleep apnea in adults with obesity.


Tirzepatide is not approved for type 1 diabetes. Compounded (non-branded) tirzepatide is not FDA-approved and may differ in dosing, purity, and safety oversight from the branded products.

Dosage


Mounjaro and Zepbound share the same titration schedule, once weekly, subcutaneous:


- Weeks 1–4: 2.5 mg (starting dose, not intended to be therapeutically effective on its own)

- Weeks 5–8: 5 mg

- Weeks 9–12: 7.5 mg

- Weeks 13–16: 10 mg

- Weeks 17–20: 12.5 mg

- Week 21 onward: 15 mg (maximum dose)


Each step is held for at least 4 weeks before increasing. Approved maintenance doses are 5 mg, 10 mg, or 15 mg — not everyone needs to reach the maximum dose to see meaningful results. If a dose is missed, it can be taken within 4 days of the missed dose; beyond that window, skip it and resume the regular weekly schedule.

Administration


Tirzepatide is injected subcutaneously once weekly, at any time of day, with or without food. Approved injection sites include the abdomen, thigh, and upper arm — rotate sites regularly to reduce irritation. Choose the same day each week for consistency; the day can be changed if needed, as long as at least 3 days have passed since the last dose.


Patients should receive proper injection training before self-administering.

Benefits

Depending on the brand and approved use, tirzepatide may help: - Improve blood sugar (A1C) control in type 2 diabetes (Mounjaro) - Support substantial, sustained weight loss — clinical trials showed average weight loss up to approximately 22.5% of body weight at 72 weeks at the maximum dose (Zepbound) - Reduce the risk of cardiovascular death or worsening heart failure in adults with HFpEF (Mounjaro) - Improve obstructive sleep apnea severity in adults with obesity (Zepbound)  Because tirzepatide activates two hormone pathways (GIP and GLP-1) rather than one, it has shown greater average weight loss in head-to-head data than GLP-1-only medications like semaglutide. Results vary by individual and depend on consistent use alongside nutrition and activity changes.

Side Effects

The most frequently reported side effects include: - Nausea - Diarrhea - Vomiting - Constipation - Abdominal pain - Injection-site reactions - Decreased appetite  These are most common during dose escalation and tend to improve as the body adjusts to each step.

Warnings

Tirzepatide carries an FDA boxed warning regarding thyroid C-cell tumor risk observed in rodent studies; it's unknown whether this risk applies to humans. It's contraindicated in patients with: - A personal or family history of medullary thyroid carcinoma (MTC) - Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)  Other important warnings include: - Pancreatitis - Acute kidney injury, generally linked to dehydration from GI side effects - Gallbladder disease - Severe gastrointestinal disease (tirzepatide may worsen existing severe GI conditions) - Serious hypersensitivity reactions - Hypoglycemia risk when combined with insulin or sulfonylureas Seek immediate medical attention for severe abdominal pain, difficulty breathing, or swelling of the face or throat.

Drug Interactions

Tirzepatide delays gastric emptying, which can affect the absorption of other oral medications — this is particularly notable with oral hormonal contraceptives, where a single dose has been shown to meaningfully reduce peak drug concentrations. Discuss alternative or additional contraception methods with your provider if this applies to you.  Other interactions requiring caution: - Insulin and sulfonylureas — increased hypoglycemia risk - Other oral medications generally — timing may need adjustment due to delayed absorption

Storage

Before first use: refrigerate at 36°F–46°F (2°C–8°C). Do not freeze.  After first use: can be kept refrigerated, or stored at room temperature (up to 86°F/30°C) for up to 21 days if refrigeration isn't available — check your specific pen's instructions, as this can vary by device. Protect from light and excessive heat. Keep out of reach of children.
