The Three Generations of GLP-1 Medications: Single, Dual, and Triple Agonists

GLP-1-class weight loss medications have developed across three generations, defined by how many hormone receptors they activate at once. First-generation single agonists, like semaglutide (Ozempic, Wegovy), activate only the GLP-1 receptor. Second-generation dual agonists, like tirzepatide (Mounjaro, Zepbound), activate both GLP-1 and GIP receptors. Third-generation triple agonists, currently represented by the investigational retatrutide, activate GLP-1, GIP, and glucagon receptors simultaneously.

How Results Compare Across Generations

Each additional receptor target has been associated with greater average weight loss in trials. A real head-to-head trial (SURMOUNT-5) directly compared dual-agonist tirzepatide against single-agonist semaglutide and found tirzepatide produced 20.2% average weight loss versus semaglutide's 13.7%. Retatrutide's Phase 3 trials have shown even greater results, up to 28.3% to 30.3% average weight loss at higher doses, though a direct head-to-head trial against tirzepatide specifically is still pending.

Why Adding Receptors Changes the Effect

Each receptor contributes a different mechanism: GLP-1 slows digestion and reduces appetite, GIP is believed to enhance insulin sensitivity and may improve tolerability, and glucagon increases energy expenditure and fat oxidation, and is linked to notable liver fat reduction in trial data. Combining mechanisms appears to produce a genuinely additive effect on results, not just a marginal improvement.

An Important Caveat Worth Knowing

Greater mechanism complexity isn't automatically a simple upgrade. Retatrutide's added glucagon activity is also linked to its distinctive dysesthesia side effect, not seen with single- or dual-agonist medications, and its overall safety profile has less real-world, post-marketing data behind it than semaglutide or tirzepatide, both of which have years of established use.

Bottom Line

More receptor targets has generally meant more powerful results in trials so far, but each generation also comes with its own distinct tradeoffs, not simply a strictly better version of the last. Retatrutide remains investigational, this generational framework describes trial data and mechanism, not yet a real, approved treatment choice.
