Who Qualifies for the Bridge Program

According to Medicare.gov, you generally qualify if you have a BMI of 35 or higher, or a BMI between 30 and 34.99 with at least one qualifying condition, including heart failure with preserved ejection fraction, uncontrolled high blood pressure, or chronic kidney disease. You must be enrolled in a Medicare Part D or Medicare Advantage plan with drug coverage, and not every plan participates, so confirming your specific plan is enrolled matters.

Important Details About the $50 Copay

The $50 flat copay does not count toward your Part D deductible or annual out-of-pocket costs, and it applies even if you receive Extra Help, the low-income subsidy, since Extra Help's cost-sharing reductions don't apply to Bridge program drugs specifically. Your prior authorization, once approved, stays valid through refills and dose changes until December 31, 2027, unless you switch to a different GLP-1 medication.

What Happens After the Bridge Program Ends

The Bridge program runs through December 31, 2027, and is intended to lead into a separate model called BALANCE, expected to begin in January 2027. Coverage beyond 2027 is not guaranteed under current program design, worth watching as the date approaches.
