The DURATION Trials: Exenatide's Foundational Diabetes Evidence

What It Is

DURATION is the clinical trial program that established extended-release exenatide, marketed as Bydureon BCise, as an effective once-weekly alternative to the original twice-daily Byetta formulation. Both brands were discontinued in October 2024, covered in detail on their respective medication pages, but this program remains the historical evidence base for how exenatide's extended-release version was developed and validated.

Key Results

DURATION-1 compared once-weekly extended-release exenatide against twice-daily immediate-release exenatide over 30 weeks, finding greater A1C reduction with the weekly formulation, 1.9 percentage points versus 1.5, with similar weight loss of about 3.6 kg for both. DURATION-2 and DURATION-5 compared extended-release exenatide against other diabetes medications, sitagliptin and pioglitazone, again finding meaningfully greater A1C reduction with exenatide.

Why the Weekly Formulation Mattered

The DURATION program's core finding, that once-weekly dosing improved both effectiveness and, presumably, convenience compared to twice-daily injections, was the direct evidence that led to Bydureon's approval and represents an early example of the broader shift toward once-weekly GLP-1 dosing that's now standard across the medication class.

Current Status

Despite this evidence base, both exenatide brands were discontinued by AstraZeneca in October 2024. The DURATION program remains useful historical context for understanding how the field arrived at once-weekly dosing as the norm.
