The EXSCEL Trial: Exenatide's Cardiovascular Evidence

What It Is
EXSCEL, Exenatide Study of Cardiovascular Event Lowering, evaluated the cardiovascular safety of extended-release exenatide, the active ingredient in the now-discontinued Bydureon BCise brand. It enrolled 14,752 adults with type 2 diabetes, 73% of whom had prior cardiovascular disease, making it one of the larger cardiovascular outcomes trials in the GLP-1 class.
Key Results
EXSCEL demonstrated cardiovascular safety, non-inferiority to placebo, but did not show a statistically significant reduction in major adverse cardiovascular events the way later trials like LEADER, SUSTAIN-6, and REWIND did for their respective medications. A post-hoc analysis found a non-significant 8% reduction in combined heart failure hospitalization and cardiovascular death.
Why This Trial's More Modest Result Matters
EXSCEL is a useful contrast point precisely because its result was less definitive than other GLP-1 cardiovascular trials, it's part of why the cardiovascular benefit seen with medications like liraglutide and semaglutide isn't assumed to be a guaranteed class effect automatically extending to every GLP-1 medication. This is also consistent with real-world data discussed elsewhere in this library, which found exenatide had a comparatively lower rate of certain gastrointestinal adverse events than some other GLP-1 medications, alongside this more modest cardiovascular result.
Current Status
Both exenatide brands, Byetta and Bydureon BCise, were discontinued by AstraZeneca in October 2024, covered in more detail on their respective medication pages.
