In the 72-week ATTAIN-1 clinical trial, adults taking Foundayo lost an average of 27 pounds at the highest studied dose. Beyond the weight-loss result itself, Foundayo's core practical benefit is accessibility: as a small molecule, it's easier to manufacture at scale than peptide-based GLP-1 medications, and its lack of dosing-ritual requirements may make it a more convenient option for people who find the administration rules of other oral GLP-1s difficult to follow consistently.
Side Effects
The most common side effects include nausea, constipation, diarrhea, vomiting, indigestion, abdominal pain, headache, abdominal swelling, fatigue, belching, heartburn, gas, and hair loss. In the ATTAIN-1 trial, nausea occurred in roughly 30 to 40% of participants at the highest dose, with vomiting and diarrhea each occurring in a similar range, most cases mild to moderate and concentrated in the first eight weeks during dose titration.
Warnings
Foundayo carries the same FDA boxed warning as other GLP-1 medications in this library, regarding the risk of thyroid C-cell tumors, including thyroid cancer, observed in rodent studies. Watch for a lump or swelling in the neck, hoarseness, trouble swallowing, or shortness of breath, and tell your provider immediately if any of these occur. Use of Foundayo alongside another GLP-1 receptor agonist medication is not recommended.
A specific warning worth knowing: there is no legal compounded version of Foundayo. Because orforglipron is a small-molecule compound, not a peptide, it cannot legally be compounded the way semaglutide or tirzepatide sometimes can under narrow exceptions. Any website selling "compounded orforglipron" is offering either an outright fraudulent product or an unapproved research chemical, not a legitimate medication. The only legitimate source is Eli Lilly, through a licensed pharmacy prescription or LillyDirect.
Drug Interactions
Foundayo has documented interactions worth discussing with your provider, including with CYP3A4-metabolized medications and simvastatin specifically. As with other GLP-1 medications, it may also affect the absorption of other oral medications due to its effects on gastric motility, though this is generally considered less pronounced than with the peptide-based oral tablets given Foundayo's different mechanism.
Storage
Store Foundayo tablets according to the manufacturer's specific instructions provided with your prescription. As an oral tablet rather than an injectable, it does not require refrigeration the way injectable GLP-1 pens do. Keep out of reach of children.
