Medication Uses
Part of the DietApp.com GLP-1 Medication Database, a comprehensive, regularly reviewed reference covering every GLP-1-class medication.
Mounjaro® (tirzepatide) is an FDA-approved dual GIP/GLP-1 receptor agonist indicated as an adjunct to diet and exercise to improve blood sugar control in adults with type 2 diabetes. Mounjaro also carries a cardiovascular indication: reducing the risk of cardiovascular death or worsening heart failure in adults with heart failure with preserved ejection fraction (HFpEF), added to the label in 2025 based on results from the SUMMIT trial, which showed a 38% reduction in this combined outcome. Mounjaro is not approved for type 1 diabetes, diabetic ketoacidosis, or chronic weight management specifically — that indication belongs to Zepbound.
Dosage
Mounjaro is injected once weekly, subcutaneously: - Weeks 1–4: 2.5 mg (starting dose, not intended for blood sugar control on its own) - Weeks 5–8: 5 mg - Weeks 9–12: 7.5 mg - Weeks 13–16: 10 mg - Weeks 17–20: 12.5 mg - Week 21 onward: 15 mg (maximum dose) Approved maintenance doses are 5 mg, 10 mg, or 15 mg once weekly, based on individual response and tolerability. If a dose is missed, it can be administered within 4 days; beyond that, skip and resume your regular schedule.
Administration
Subcutaneous injection, once weekly, any time of day, with or without food. Inject into the abdomen, thigh, or upper arm, rotating sites to reduce irritation. If also using insulin, administer as a separate injection — never mix the two products.
Benefits
When used as prescribed, Mounjaro may help: - Lower blood sugar (A1C) levels in adults with type 2 diabetes - Reduce the risk of cardiovascular death or worsening heart failure in adults with HFpEF - Support meaningful weight loss as a secondary effect, though Mounjaro is not FDA-approved specifically for weight management (that indication belongs to Zepbound)
Side Effects
The most frequently reported side effects include: - Nausea - Diarrhea - Vomiting - Constipation - Decreased appetite - Injection-site reactions Most noticeable during dose escalation, often improving with continued use.
Warnings
Mounjaro carries an FDA boxed warning regarding thyroid C-cell tumor risk observed in rodent studies. Contraindicated in patients with: - A personal or family history of medullary thyroid carcinoma (MTC) - Multiple Endocrine Neoplasia syndrome type 2 (MEN 2) Other important warnings include: - Pancreatitis - Acute kidney injury, typically linked to dehydration from GI side effects - Gallbladder disease - Diabetic retinopathy complications - Serious hypersensitivity reactions Seek immediate medical attention for severe abdominal pain, difficulty breathing, or facial/throat swelling.
Drug Interactions
Particular caution is needed with: - Insulin and sulfonylureas — increased hypoglycemia risk; a dose reduction may be needed - Oral medications generally — Mounjaro delays gastric emptying, which can affect absorption - Oral hormonal contraceptives — Mounjaro has been shown to meaningfully reduce peak drug concentrations; discuss alternative contraception with your provider Never share a Mounjaro pen between patients, even with a new needle.
Storage
Before first use: refrigerate at 36°F–46°F (2°C–8°C). Do not freeze. After first use: refrigerate, or store at room temperature (up to 86°F/30°C) for up to 21 days. Protect from light and heat. Keep out of reach of children.
