Trulicity® (dulaglutide) is an FDA-approved GLP-1 receptor agonist indicated as an adjunct to diet and exercise to improve blood sugar control in adults and children 10 years and older with type 2 diabetes. It's also approved to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes who have either established cardiovascular disease or multiple cardiovascular risk factors.

Trulicity is not approved for type 1 diabetes, diabetic ketoacidosis, or chronic weight management. Weight loss with Trulicity tends to be modest compared with semaglutide or tirzepatide, so it's not typically prescribed off-label for weight loss the way some other GLP-1s are.

Dosage

Trulicity is injected once weekly, subcutaneously. The starting dose is 0.75 mg once weekly, which can be increased to 1.5 mg, 3 mg, or a maximum of 4.5 mg once weekly based on individual response and glycemic needs. Trulicity comes in a single-dose pre-filled pen or syringe at each strength. If a dose is missed, it can be taken within 3 days; beyond that, skip it and resume the regular schedule.

Administration

Subcutaneous injection, once weekly, at any time of day, with or without food. Inject into the abdomen, thigh, or upper arm, rotating sites to reduce irritation.

Benefits

When used as prescribed, Trulicity may help:

Side Effects

The most frequently reported side effects include nausea, diarrhea, vomiting, abdominal pain, and decreased appetite. These are most common during the first weeks of treatment or after a dose increase, and typically improve over time.

Warnings

Trulicity carries an FDA boxed warning regarding thyroid C-cell tumor risk observed in rodent studies. It's contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Other important warnings include pancreatitis, acute kidney injury (typically linked to dehydration from GI side effects), serious hypersensitivity reactions, and diabetic retinopathy complications. Seek immediate medical attention for severe abdominal pain, difficulty breathing, or facial/throat swelling.

Drug Interactions

Particular caution is needed with insulin and sulfonylureas, which carry increased hypoglycemia risk when combined with Trulicity. Trulicity also delays gastric emptying, which can affect the absorption of other oral medications.

Storage

Before first use, refrigerate at 36°F–46°F (2°C–8°C). Do not freeze. After first use, can be kept at room temperature (up to 86°F/30°C) for up to 14 days if needed. Protect from light. Keep out of reach of children.
