Zepbound® (tirzepatide) is an FDA-approved dual GIP/GLP-1 receptor agonist indicated for chronic weight management, used together with a reduced-calorie diet and increased physical activity. Approved uses include: - Adults with obesity (BMI ≥30 kg/m²) - Adults who are overweight (BMI ≥27 kg/m²) with at least one weight-related condition, such as hypertension, type 2 diabetes, or dyslipidemia - Moderate-to-severe obstructive sleep apnea in adults with obesity — a distinct FDA approval separate from the weight-management indication Zepbound is not approved for type 1 diabetes. Zepbound and Mounjaro should never be used together, since they contain the same active ingredient.
Dosage
Zepbound is injected once weekly, subcutaneously, using the same titration schedule as Mounjaro: - Weeks 1–4: 2.5 mg - Weeks 5–8: 5 mg - Weeks 9–12: 7.5 mg - Weeks 13–16: 10 mg - Weeks 17–20: 12.5 mg - Week 21 onward: 15 mg (maximum dose) Approved maintenance doses are 5 mg, 10 mg, or 15 mg — reaching the maximum dose isn't required for meaningful results; the 5 mg maintenance dose alone produced approximately 15% average weight loss at 72 weeks in clinical trials. Missed doses can be taken within 4 days; otherwise, skip and resume your regular schedule.
Administration
Subcutaneous injection, once weekly, any time of day, with or without food. Inject into the abdomen, thigh, or upper arm, rotating sites regularly to reduce irritation.

Side Effects
The most frequently reported side effects include: - Nausea - Diarrhea - Vomiting - Constipation - Abdominal pain - Injection-site reactions - Decreased appetite Most noticeable during dose escalation, often improving with continued use.
Warnings
Zepbound carries an FDA boxed warning regarding thyroid C-cell tumor risk observed in rodent studies. Contraindicated in patients with: - A personal or family history of medullary thyroid carcinoma (MTC) - Multiple Endocrine Neoplasia syndrome type 2 (MEN 2) Other important warnings include: - Pancreatitis - Acute kidney injury, typically linked to dehydration from GI side effects - Gallbladder disease - Severe gastrointestinal disease — Zepbound may worsen existing severe GI conditions - Serious hypersensitivity reactions Seek immediate medical attention for severe abdominal pain, difficulty breathing, or facial/throat swelling.
Drug Interactions
Particular caution is needed with: - Insulin and sulfonylureas — increased hypoglycemia risk - Oral hormonal contraceptives — Zepbound has been shown to meaningfully reduce peak drug concentrations; discuss alternative contraception with your provider - Oral medications generally — delayed gastric emptying can affect absorption
Storage
Before first use: refrigerate at 36°F–46°F (2°C–8°C). Do not freeze. After first use: refrigerate, or store at room temperature (up to 86°F/30°C) for up to 21 days. Protect from light and heat. Keep out of reach of children.
