What It Is
Diarrhea means loose, watery, or more frequent stools than usual. Real-world data from the NIH's All of Us cohort found diarrhea in roughly 33% of GLP-1 users, making it one of the most common GI side effects alongside nausea and constipation, sometimes even in the same patient at different points in treatment.
Why It Happens
GLP-1 medications alter gut motility, and while the more commonly discussed effect is slowed emptying, the change in digestive rhythm can swing toward diarrhea in some patients rather than constipation. Some reporting suggests longer-acting GLP-1 formulations (like weekly injections) trend toward more diarrhea, while shorter-acting, mealtime-dosed medications trend more toward nausea and vomiting, though individual response varies significantly.
When to Seek Care
Mild, short-lived diarrhea during dose escalation is common. Contact your provider if diarrhea is severe, lasts more than a couple of days, or is accompanied by signs of dehydration (dizziness, reduced urination, extreme thirst). Seek prompt care if you notice blood in your stool, which is not a typical GLP-1 side effect and warrants evaluation.
How to Manage It
Standard management approaches include:
- Staying well-hydrated, since diarrhea increases fluid loss
- Avoiding high-fat, greasy, or very high-fiber foods temporarily if diarrhea is active, since these can worsen it
- Replacing electrolytes if diarrhea is prolonged, particularly sodium and potassium
- Talking to your provider if diarrhea consistently follows each dose increase, since a slower titration schedule may help
