Constipation

What It Is
Constipation means having fewer bowel movements than usual, or stools that are hard and difficult to pass. Reported rates on GLP-1 medications range from about 24% to 30% depending on the specific drug and study population, according to real-world data from the NIH's All of Us cohort.
Why It Happens
The same slowed gastric emptying that contributes to nausea and appetite suppression also slows movement through the rest of the digestive tract. Reduced food and fiber intake, a natural consequence of eating less on these medications, compounds the effect, since fiber is what normally keeps stool moving. Dehydration, sometimes from other GI side effects like nausea or reduced fluid intake, makes it worse.
When to Seek Care
Occasional, mild constipation is common and usually manageable with diet and hydration changes. Contact your provider if you haven't had a bowel movement in several days, if you have severe abdominal pain or bloating, or if constipation is accompanied by vomiting, these can indicate a more significant slowdown in digestion that needs evaluation.
How to Manage It
Clinical guidance for GLP-1-related constipation centers on a few consistent recommendations:
- Increase fiber intake gradually (adding fiber too quickly can worsen bloating)
- Increase water intake throughout the day, not just at meals
- Regular physical activity, even light walking, helps stimulate digestion
- Discuss an over-the-counter stool softener or fiber supplement with your provider if diet changes aren't enough
- Avoid straining, which can worsen the problem over time
