What It Is

Fatigue is a persistent sense of tiredness or low energy that doesn't fully resolve with rest. It's a commonly reported experience on GLP-1 medications, though estimates of how many people experience it vary more widely across sources than the GI side effects do.

Why It Happens

Unlike nausea or constipation, fatigue on GLP-1 medications isn't a direct pharmacological effect, it's typically downstream of other changes. The most common contributors are: significantly reduced calorie intake (your body is running on less fuel), dehydration from GI side effects like diarrhea or reduced fluid intake, and the general physiological adjustment to rapid weight loss. For some patients, disrupted sleep from GI discomfort also plays a role.

When to Seek Care

Mild fatigue that improves as your body adjusts to the medication and to eating less is common, especially in the first several weeks. Discuss fatigue with your provider if it's severe, persistent beyond the initial adjustment period, or accompanied by other symptoms like dizziness, rapid heartbeat, or fainting, these can indicate dehydration or another issue that needs evaluation rather than something to push through.

How to Manage It

Since fatigue is usually secondary to other factors, management focuses on those root causes:

  • Prioritize protein intake even when appetite is low, since inadequate protein can worsen fatigue and contribute to muscle loss
  • Stay consistently hydrated throughout the day, not just when thirsty
  • Address any GI side effects (nausea, diarrhea) that may be disrupting sleep or nutrition
  • Avoid skipping meals entirely, even if portions are smaller than before
  • Talk to your provider if fatigue is significant, it's worth ruling out other causes, including low blood sugar if you're also managing diabetes
