What It Is
Vomiting is the forceful expulsion of stomach contents. It's less common than nausea alone but still a frequently reported GLP-1 side effect, one large clinical trial dataset found vomiting in roughly 27% of GLP-1 users compared to about 4% on placebo.
Why It Happens
Vomiting shares the same underlying mechanisms as nausea, slowed gastric emptying combined with direct action on brainstem nausea centers, but represents a more intense version of that response. It's most common during the first weeks of treatment or shortly after a dose increase, when the digestive system hasn't yet adapted.
When to Seek Care
Occasional vomiting during dose escalation, especially if it resolves within a day, is a known pattern. Seek prompt medical attention if you can't keep any fluids down for more than 24 hours (risk of dehydration), if vomiting is accompanied by severe abdominal pain (possible pancreatitis), or if you see blood in vomit. Persistent vomiting is one of the clearer signals that a dose may need to be reduced or escalation paused, discuss this with your provider rather than pushing through.
How to Manage It
Management overlaps closely with nausea management:
- Smaller, more frequent meals rather than large portions
- Avoid high-fat, greasy, or very sugary foods, which are harder to digest
- Stay hydrated with small, frequent sips rather than large amounts at once if nausea is present
- Talk to your provider about a slower dose-escalation schedule if vomiting recurs with each increase
