The SURMOUNT-OSA Trial: Tirzepatide's Sleep Apnea Evidence

What It Is

SURMOUNT-OSA is the clinical trial that established Zepbound's FDA approval for treating obstructive sleep apnea, distinct from its weight-management approval. It was actually two parallel 52-week studies, Study 1 enrolled participants unable or unwilling to use positive airway pressure (PAP) therapy, while Study 2 enrolled participants already using PAP therapy who planned to continue it during the trial.

Key Results

The trial measured improvement in the apnea-hypopnea index, the standard measure of OSA severity, using overnight sleep studies. Participants received tirzepatide gradually titrated up to their maximum tolerated dose (10 or 15 mg) alongside lifestyle intervention, or placebo. The trial demonstrated meaningful improvement in sleep apnea severity, forming the direct evidence base for Zepbound's distinct OSA indication.

A Genuinely Novel Trial Design

SURMOUNT-OSA specifically investigated whether tirzepatide could improve OSA by targeting its underlying cause, excess weight and tissue around the airway, rather than only managing symptoms the way PAP therapy does. The trial also used digital wearables and home sleep testing to capture more detailed data on how quickly improvement occurred and how it affected daily physical activity.

Why Two Separate Studies Mattered

Testing both people not currently using PAP therapy and people already using it separately let researchers understand tirzepatide's effect both as a standalone treatment and as a genuine complement to existing PAP therapy, rather than assuming it would work the same way in both situations.
