What It Is

SURMOUNT is the clinical trial program that established tirzepatide's effectiveness for chronic weight management. SURMOUNT-1, published in NEJM in 2022, is the pivotal trial in the series and formed the primary basis for Zepbound's FDA approval.

Key Results

SURMOUNT-1 showed average weight loss of up to 20.9% of body weight at the highest studied dose over 72 weeks, a result researchers have described as the strongest single outcome in obesity pharmacotherapy to date at the time of publication. Later, SURMOUNT-5 became the first direct head-to-head trial comparing tirzepatide against semaglutide, finding tirzepatide produced significantly greater weight loss, 20.2% versus 13.7% at 72 weeks.

Side Effects Observed

Gastrointestinal effects, primarily mild to moderate nausea, were the dominant side effects across the SURMOUNT trials, consistent with what's seen across the GLP-1 and dual-agonist medication class generally.

A Real-World Perception Gap

A survey of 122 physicians found their estimated average patient weight loss (9.22%) was notably lower than what STEP and SURMOUNT trials actually reported (14.9% and 18.5% respectively), and physicians also underestimated trial-reported side effect rates. This is worth knowing, real-world outcomes and even provider expectations can diverge from published trial averages in either direction, which is part of why individual results genuinely vary.
