Semaglutide Side Effects: What Patients Need to Know

Common Side Effects

  • Nausea — most common, especially early in treatment

  • Vomiting

  • Diarrhea

  • Constipation

  • Abdominal pain

  • Headache

  • Fatigue

  • Injection-site reactions (injection form only)

These cluster around dose increases. Symptoms often lessen within the first several weeks at a given dose.

Less Common but Important Side Effects

  • Dehydration from persistent GI symptoms

  • Low blood sugar (hypoglycemia) — increased risk with insulin or a sulfonylurea

  • Increased resting heart rate — average increases of 1–4 bpm with Wegovy

  • Gallbladder problems, including gallstones

  • Diabetic retinopathy complications (Ozempic) — monitor if you have prior retinopathy

Serious Warnings You Should Know About

Boxed warning — thyroid tumors. Both carry the FDA's most serious label warning: rodent studies showed thyroid C-cell tumors, including MTC. Contraindicated for anyone with a personal or family history of MTC or MEN 2. Tell your provider immediately about a neck lump, hoarseness, trouble swallowing, or shortness of breath.

Pancreatitis. Seek immediate care for severe, persistent abdominal pain.

Kidney injury. Generally linked to dehydration from GI side effects.

Serious allergic reactions. Facial/lip/tongue swelling or difficulty breathing require emergency care.

Mood changes. Discuss new or worsening suicidal thoughts or behavior with your provider promptly.

When to Call Your Doctor vs. Seek Emergency Care

Contact your provider if: GI symptoms persist, you notice a new neck lump or hoarseness, or mood changes appear.

Seek emergency care if: severe abdominal pain doesn't resolve, signs of serious allergic reaction appear, or low blood sugar symptoms don't resolve.
