Tirzepatide Side Effects: What Patients Need to Know
Whether you've been prescribed Mounjaro® or Zepbound®, knowing what side effects to expect — and which need immediate attention — makes treatment far less stressful.
For background, see What Is Tirzepatide? and Mounjaro vs. Zepbound.

Common Side Effects
Nausea — most common, especially during dose escalation
Diarrhea
Vomiting
Constipation
Abdominal pain
Decreased appetite
Injection-site reactions
These cluster around dose increases, which is why both medications use a gradual titration schedule. Symptoms often lessen within a few weeks at each dose level.
Less Common but Important Side Effects
Dehydration from persistent GI symptoms
Low blood sugar (hypoglycemia) — increased risk with insulin or a sulfonylurea
Gallbladder problems
Worsening of severe existing GI conditions — tirzepatide can exacerbate conditions like severe gastroparesis
Serious Warnings You Should Know About
Boxed warning — thyroid tumors. Both Mounjaro and Zepbound carry the FDA's most serious label warning: rodent studies showed thyroid C-cell tumors. Contraindicated for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2. Tell your provider immediately about a neck lump, hoarseness, or trouble swallowing.
Pancreatitis. Seek immediate care for severe, persistent abdominal pain.
Kidney injury. Generally linked to dehydration from GI side effects — staying hydrated helps reduce this risk.
Serious allergic reactions. Facial/lip/tongue swelling or difficulty breathing require emergency care.
When to Call Your Doctor vs. Seek Emergency Care
Contact your provider if: GI symptoms are persistent, you notice a new neck lump or hoarseness, or symptoms of gallbladder issues appear.
Seek emergency care if: severe abdominal pain doesn't resolve, or you have signs of a serious allergic reaction.
Why Tracking Side Effects Matters
Logging symptoms alongside your dose and timing makes it much easier to spot whether something is titration-related and temporary, or worth flagging sooner. DietApp tracks dose, symptoms, hydration, and timing in one place.
